After Dental Implant Placement in Philadelphia

Caring for Your Implant Site Right After Surgery

After dental implant surgery in Philadelphia, what you do in the first 24 to 48 hours has a direct impact on how well your implant integrates with the jawbone. Proper post-op care reduces the risk of infection, supports osseointegration, and helps you recover with minimal discomfort. Whether you have received a single-tooth implant, bone grafting, or a full-arch restoration at our Philadelphia practice, these instructions apply to the period immediately following your procedure. For more detail on your specific treatment, visit our dental implants overview page.
At Laudenbach Periodontics & Dental Implants, Dr. Jay Laudenbach is a Board Certified diplomate of the American Board of Periodontology and places Straumann implants, the leading titanium implant system in the world. The guidance below reflects his clinical approach to post-operative care and is designed to help you protect your investment and your healing tissue during the critical first weeks.
Do not disturb the implant site on the day of surgery. Avoid rinsing, spitting, or touching the surgical area for the first 24 hours. A metal healing abutment may protrude through the gum tissue, which is normal. Some bleeding or redness in the saliva is expected for the first day.

Managing bleeding and swelling after implant surgery

Bleeding after implant placement is normal and may continue for several hours. Fold a piece of clean gauze and apply firm pressure directly over the surgical site for 30 minutes. Keep your head elevated and avoid spitting or sucking through a straw. Pink or blood-tinged saliva is expected for up to 48 hours. If heavy bleeding continues beyond 1 hour of direct pressure, contact our Philadelphia office.

Swelling reaches its peak at 48 to 72 hours after surgery, then gradually decreases. Apply an ice pack to your cheek in the area of the surgery for 20 minutes on, 20 minutes off during the first 48 hours. After 48 hours, switch to a warm compress to help reduce remaining swelling. Some bruising on the cheek or jaw is normal and will resolve within 7 to 10 days.

Diet and activity after dental implant surgery

Start with liquids and very soft foods immediately after surgery, including yogurt, applesauce, soup, pudding, and scrambled eggs. Avoid chewing on the implant site for at least 2 weeks. Progress to solid foods gradually as healing allows, and avoid hard, crunchy, or chewy foods during the 10 to 12-week osseointegration period.
Rest for the first 24 to 48 hours and avoid strenuous physical activity, including exercise. Elevated heart rate increases blood flow to the surgical area, which can prolong bleeding. Avoid tobacco and alcohol for at least 10 days after surgery, as both interfere with tissue healing and increase the risk of implant failure. You can review more about the healing process on our single tooth dental implants and bone grafting pages.

Oral hygiene during implant healing

Do not rinse your mouth on the day of surgery. Beginning the morning after your procedure, gently rinse with warm salt water 2 to 3 times daily. Dissolve half a teaspoon of salt in 8 ounces of warm water and hold it gently in your mouth without vigorous swishing. If chlorhexidine rinse (Peridex) was prescribed, use it as directed.

Brush your other teeth normally starting the day after surgery, but avoid brushing directly over the implant site or sutures for the first week. Use a soft-bristle toothbrush around the adjacent area. Do not use a Waterpik or oral irrigator for at least 12 weeks following surgery. According to the American Academy of Periodontology, consistent oral hygiene is critical to long-term implant success. Our team will review your hygiene protocol at your follow-up appointment.

Pain management and medication after implant placement

Most patients experience manageable discomfort in the first 2 to 3 days after implant surgery. Begin taking ibuprofen (Advil or Motrin) before the local anesthetic wears off, taking 400 to 600 mg every 6 hours with food to control most post-operative pain effectively. If a prescription pain medication was provided, take it as directed and avoid driving or operating machinery while using it.

If antibiotics were prescribed, take the full course as directed even if you feel better before it is finished. A rash, hives, or difficulty breathing may indicate an allergic reaction; discontinue the medication and seek immediate care. Discomfort should decrease noticeably each day after the first 48 hours. Contact us if pain is worsening after day 3. For patients curious about periodontal maintenance following implant integration, we provide comprehensive ongoing care at our Philadelphia practice.

Warning signs to watch for after surgery

Contact our office if you experience any of the following after your dental implant surgery in Philadelphia: heavy bleeding not controlled by gauze pressure after 1 hour, a fever above 101°F, severe or worsening swelling after 72 hours, a foul taste or discharge from the wound, or any sign the implant is moving or feels loose. Numbness or tingling in the lip, chin, or tongue after surgery should be reported promptly, as this may indicate nerve involvement and should be evaluated quickly to allow for the most appropriate management.

Most complications can be addressed successfully when caught early. We are available by phone at (215) 985-4337 during business hours, and our team will advise you on whether you need to be seen sooner. Never ignore post-operative symptoms that feel abnormal, as early communication leads to better outcomes.
